Membership Guidelines

  • Memberships are valid for 12 months from enrollment.
  • Memberships cannot be combined with insurance or third-party financing.
  • Unused services do not roll over and have no cash value.
  • Membership fees are non-refundable, non-transferable, and cannot be prorated.
  • Discounts apply only while the membership is active and in good standing.
  • Payment for all services is expected at the time of service unless prepaid.
  • Services are provided as clinically necessary based on professional judgment.
  • Discounts do not apply to retail products or merchandise.
  • A minimum of 48 hours’ notice is required for appointment changes or cancellations.
  • Smilestry reserves the right to modify membership benefits, pricing, and discounts.
